Nurse at Home provides health-care management and guidance as well as hands-on services to members of the Okanagan community. We remain patient-driven, focusing on the right choices for our clients in order to ensure they receive the best possible care. We spend the time with clients and family members to ensure we provide them with a care package designed to fit their health and lifestyle needs, and we continue close monitoring to be able to promptly address any health changes.
Nurse at Home can become involved at critical times or to provide on-going support. Our clients can rely on us as a single point-of-contact, offering health-care guidance, advocacy and support services as required. - development of comprehensive care plans and management of care
- in home health assessment and safety check
- ongoing assessments and monitoring
- medication review, management and monitoring
- pre and post operative care including wound care and dressing changes
- diabetes management, education, medication administration and teaching
